December News Bites

  • Don’t miss out on $450! 31 is the last day to complete the Health Management program to earn $450 off your 2017 medical premiums. At a minimum, be sure to complete the Health Risk Questionnaire (HRQ) so you’re eligible for other incentives and to avoid paying an additional $100 for medical coverage next year. You can take the HRQ and see your progress on all the incentives at ra.staywell.com.
  • Here’s one holiday card you probably won’t get: a new medical/prescription ID card. In most cases, you’ll continue to use your current medical/prescription drug ID card in 2017. You’ll receive new ID cards in December only if you’ve switched medical plans, changed your covered dependents or changed your address. Keep in mind: Employees who cover dependents will receive two cards in total. If you don’t receive your ID card this month or if you need addition copies, you can print a card online or use the mobile app to view, fax or email your ID card.
  • No need for vision ID cards. UHC vision providers don’t require ID cards because they can verify your coverage with your name and date of birth. However, if you or a family member would prefer to have one, you can print it from a link on the vision portal home page or view it on your smartphone.   • The Rockwell Automation Service Center and the Rockwell Automation Service Center at Fidelity will be closed Dec. 26 (the day after Christmas) and Jan. 2 (the day after New Year’s Day).
